Failed Root Canal Symptoms & Detection

Young woman experiencing dental pain and holding her jaw, a common symptom of a failed root canal. Persistent pain, swelling, and sensitivity can indicate infection or treatment failure.

What Is a Failed Root Canal?

A failed root canal can make you worry if you keep feeling pain, swelling, or sensitivity that won’t go away. When this happens, it’s crucial to get the right diagnosis and treatment from a specialist who knows what they’re doing. Dr. Alavy at Hemlock Dental Clinic is an expert at spotting and fixing root canals that have gone wrong. She aims to save your tooth when she can and make you feel better. If you think you have faced a failed root canal, give our office a call at 604-738-9202 today. Book a check-up with Dr. Alavy to get a clear idea of what to do next.

What Are the Symptoms of a Failed Root Canal?

Recognizing the symptoms early can help you avoid complications. Common signs of a failed root canal include:

  • Persistent pain or pressure near the treated tooth
  • Swelling or tenderness in the gums
  • A visible pimple or bump on the gum
  • Discoloration of the tooth
  • Sensitivity to hot or cold
  • Bad taste or odor in the mouth

If you’re experiencing any of these symptoms, visit a walk-in dental clinic in Vancouver or consult with Dr. Alavi at our general dentistry clinic as soon as possible.

Not all discomfort means failure—sometimes it’s part of the normal root canal recovery time.

Why Do Root Canals Fail?

Root canal failure can be caused by multiple factors:

  • Missed or untreated canals
  • Complicated root canal anatomy
  • Cracks or fractures in the tooth
  • Delayed or poor placement of the crown
  • New decay or trauma

Good Root Canal After Care can lower the risk of reinfection—avoid smoking, keep the area clean, and follow your dentist’s instructions.

What Happens Next When a Root Canal Fails?

When a root canal fails, there are several treatment options available depending on the severity of the issue:

Condition Possible Next Steps
Mild discomfort or tenderness Monitoring, pain management
Infection or abscess Endodontic retreatment
Structural damage (e.g., crack) Tooth extraction and replacement (e.g., dental implant)

Failed Root Canal Symptoms can vary from mild discomfort to infection or tooth damage, with treatment options ranging from monitoring to retreatment or replacement.

What Is the Root Canal Failure Rate?

Root canal treatment generally has a high success rate, although outcomes can vary depending on how success is defined, the condition of the tooth, and the length of follow-up. Regular follow-up with your dentist or endodontist can help monitor healing and identify possible complications early.

Antibiotics after root canal treatment are not routinely recommended for normal post-treatment soreness. They may be prescribed when there are signs of a spreading infection or systemic involvement, such as increasing swelling, fever, or malaise. However, antibiotics cannot correct the underlying cause of a failed root canal, so persistent or worsening symptoms require professional evaluation.

What Causes Root Canals to Fail?

  • A failed root canal can happen when bacteria are able to re‑enter or remain inside the tooth. This typically occurs because of one or more of these reasons:
    1. Not cleaning or sealing the canals
    2. Bacteria getting back in through gaps around the crown or filling
    3. Fresh bacterial infection from tooth decay or cracks
    4. Lack of good oral care or skipping check-ups after the procedure

Is It Possible to Repair a Failed Root Canal?

In many cases, a failed root canal can be successfully treated and the tooth can still be saved. The most common approach is endodontic retreatment, where the dentist removes the old filling material, thoroughly cleans and disinfects the canals again, and then seals them carefully. Other treatment options may include:

  • Apicoectomy (surgically removing the tip of the root and sealing it).
  • Extraction followed by dental implant placement, if the tooth is too damaged to be restored.

Can You See a Failed Root Canal on an X-ray?

Many failed root canals can be seen on an X‑ray, but not all of them. On a healthy and well‑healed tooth, the root looks clean and the bone around the tip are solid and even. When a root canal has failed, your dentist can see dark shadowy areas at the root tip, which usually mean there is still infection or an abscess.

They might also notice unfilled or missed canals, where parts of the root look empty instead of filled, or signs of bone loss around the root, showing that the body is struggling to clear the infection. If that dark area stays the same size or gets bigger on X‑rays, it means the problem has not healed.

However, X‑rays are not perfect and have some limitations. Very early or small infections, tiny cracks, or certain types of damage may not show clearly on a regular 2D image. In those cases, your dentist may recommend a 3D scan (CBCT) and a full exam to confirm whether the root canal has truly failed. If you need help, contact us now.

Key Indications of a Failed Root Canal on an X-ray:

  • Dark Areas at the Root Tip: Shadows or dark spots, which indicate an ongoing infection or an abscess that has formed despite the treatment.
  • Unfilled or Missed Canals: The X-ray may show empty spaces or gaps in the filling material where bacteria could thrive
    .
  • Bone Loss: Thinning or erosion of the bone around the tooth, indicating that the body is failing to fight the infection.
  • Unresolved Infection: A persistent shadow that does not diminish or shrink over time (often checked 6-12 months post-treatment).

Limitations:

While X-rays are highly effective, they cannot always detect infections in the very early stages or show all types of damage. Sometimes, a small, persistent infection may be missed on a 2D X-ray, leading to the need for a 3D CBCT scan for a more detailed

Should the Dentist Pay for a Failed Root Canal?

Responsibility depends on the cause. If the failure is due to negligence or improper technique, the dentist may offer retreatment at a reduced cost or for free. However, many failures occur due to unpredictable biological factors.

Can a Root Canal Become Infected Again?

Yes. Even after a successful treatment, reinfection is possible if bacteria re-enter the tooth. That’s why aftercare and follow-up visits at a reliable general dentistry clinic are essential.

I had a dental emergency. The dentist cancelled her own lunch to accommodate me (she never told me this but the front desk confirmed it). The dentist listened to my concerns and expertly fixed the problem. Honestly, I would give more than 5 stars if I could. I'm really grateful!

The first time I visited this clinic I showed up when they were closing, Zhina kindly let me in and Dr. Alavi checked my teeth patiently. After that, Dr. Alavi and her great assitants Maryam and Anandi, fixed my prolonged and difficult teeth problem in several very pleasant appointments. Dr, Alavi's skill and expertise are impeccable and most importantly, she really cares about her patients. Niki also always goes out of her way to help me out, doing extra work to make sure I get the best for my teeth, she also speaks madarin! Pendar is good at cleaning teeth too. Can't wait for my teeth to rot and I can give this great clinic another visit! ha
